This form requests basic information about the club including meeting days, times, locations, and club officer's and advisor's information. There is also a hyperlink to the ICC Bylaws in case they need to be reviewed (these will be updated over the summer semester). It is imperative that you fill out this form in order for your club to continue as a chartered club.
This completion of this form signifies the certification of club presidents and advisors. Upon completion, it will be submitted to the Coordinator of Clubs and Service for review. Once approval has been received, the club president and advisor are certified for the 2017-2018 school year.

This form will notify our Involvement Coordinator that you are interested in being involved with DSUSA. This person will contact you and provide you with information. The Blazer Digest is primarily for the publication of campus wide events hosted by DSUSA. Other entities/departments on campus are welcome to submit events that are student oriented and applicable to the entire student body. Inclusion of events in the Blazer Digest is at the discretion of the DSUSA PR Branch. Depending on layout and design, space availability, and other factors, your event may, or may not, be posted in the Blazer Digest. Form submissions will be evaluated every Monday on a weekly basis. Blazer Digest requests for recurring events must be resubmitted for each occurrence.
Events not submitted 2 weeks in advance will not be published.
